Experienced Home Health Licensed Practical Nurse – Blacksburg and Surrounding Area

We are currently hiring for Part-time and PRN positions.

Responsibilities:

  • Conduct comprehensive nursing assessments of patients in their homes, including physical, psychosocial, and environmental factors, to develop individualized care plans.

  • Perform skilled nursing procedures and treatments as prescribed by the physician or advanced practice provider, such as wound care, medication administration, injections, catheter care, and monitoring vital signs.

  • Monitor patients' health status, symptoms, and response to treatment, and communicate any changes or concerns to the supervising registered nurse or healthcare provider.

  • Educate patients and caregivers on disease management, medication adherence, self-care techniques, and preventive measures to promote optimal health and independence.

  • Provide emotional support, counseling, and reassurance to patients and families coping with illness, disability, or end-of-life issues, demonstrating empathy, compassion, and cultural sensitivity.

  • Coordinate care with other members of the healthcare team, including physicians, therapists, social workers, and home health aides, to ensure holistic and coordinated care delivery.

  • Document nursing assessments, interventions, and patient responses accurately and in accordance with regulatory standards and organizational policies, utilizing electronic health record (EHR) systems.

  • Collaborate with patients, families, and caregivers to develop realistic goals and discharge plans, facilitating transitions of care and continuity of services as patients' needs evolve.

  • Implement infection control practices, safety measures, and emergency protocols to protect patients, caregivers, and oneself from harm, adhering to established guidelines and protocols.

  • Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives to review patient progress, identify barriers to care, and enhance care coordination and outcomes.

Qualifications:

  • Graduate of an accredited practical/vocational nursing program with a current license to practice as an LPN or LVN in the state of employment.

  • Minimum of 2–3 years of nursing experience, preferably in home health, community health, or a related healthcare setting.

  • Strong clinical assessment skills, critical thinking abilities, and proficiency in administering medications, treatments, and procedures safely and effectively.

  • Excellent communication, interpersonal, and patient education skills, with the ability to establish rapport with patients and families from diverse backgrounds.

  • Knowledge of nursing principles, practices, and standards of care relevant to home health nursing, including regulatory requirements and best practices.

  • Ability to work autonomously, manage time effectively, and prioritize tasks in a dynamic environment.

  • Proficiency in using electronic health record (EHR) systems and other technology platforms.

  • Valid driver’s license, reliable transportation, and willingness to travel to patients’ homes within the designated service area.
