What You’ll Do:

• Provide home health occupational therapy services to adult and geriatric patients
• Perform evaluations and develop individualized treatment plans
• Focus on activities of daily living (ADLs), safety, and functional independence
• Recommend adaptive equipment and home modifications
• Educate patients and caregivers on techniques and safety
• Collaborate with physicians and interdisciplinary care teams
• Complete accurate and timely EMR documentation

What We’re Looking For:

• Licensed Occupational Therapist (OT) in Virginia 
• Minimum of 1–2 years of clinical experience required
• Home health experience preferred, not required
• Strong communication, organization, and time management skills
• Ability to work independently and travel between patient homes
