Position Summary

Provide skilled physical therapy services to patients in their homes. Enjoy flexible scheduling, clinical autonomy, and strong interdisciplinary collaboration while making a meaningful impact in your community.

Key Responsibilities

• Perform comprehensive PT evaluations

• Develop and adjust individualized treatment plans

• Deliver skilled therapy in the home setting

• Complete accurate, timely documentation

• Collaborate with nurses, aides, and physicians

• Educate patients and caregivers on safety, mobility, and exercises

Qualifications

• Active Virginia PT license

• Minimum 1 year PT experience (home health preferred)

• Strong clinical judgment and communication skills

• Ability to work independently

• Valid driver’s license & reliable transportation
