Responsibilities

• Perform in-home nursing assessments

• Develop and manage individualized care plans

• Provide skilled nursing care (wound care, medication management, infusions, catheter care, palliative care)

• Monitor patient status and report changes

• Educate patients and caregivers

• Coordinate care with interdisciplinary teams

• Document care in EHR systems

Qualifications

• Active RN license (MD)

• 2–3 years nursing experience (home health preferred; LPN experience considered)

• Strong assessment and clinical skills

• Wound care experience preferred

• Valid driver’s license and reliable transportation
