Position: Experienced Licensed Master Social Worker – Washington, DC
Responsibilities:
-
Conduct comprehensive psychosocial assessments of patients and families to identify strengths, challenges, and support systems affecting health and well-being.

Provide crisis intervention, counseling, and emotional support to patients and families facing medical diagnoses, end-of-life issues, and life transitions.

Collaborate with the interdisciplinary healthcare team to develop and implement individualized care plans.

Advocate for patients’ rights and connect them with appropriate resources such as financial assistance, housing, and community support services.

Guide patients and families in navigating the healthcare system, medical procedures, and treatment options.

Facilitate effective communication between patients, families, providers, and community agencies to ensure continuity of care.

Accurately document assessments, interventions, and case management activities in compliance with regulations and policies.

Participate in case conferences, discharge planning, and multidisciplinary team meetings.
-
Remain current on social, economic, and legislative issues affecting patient care and apply knowledge in practice.

Engage in professional development and maintain ethical, trauma-informed, and culturally competent practice.
Qualifications:
-
Master’s degree in Social Work (MSW) from a CSWE-accredited program.

Current state licensure or eligibility as an LMSW or LCSW.

2–3 years of clinical experience in medical social work, healthcare, or related fields.

Strong knowledge of psychosocial assessment, counseling, and case management practices.

Familiarity with community resources, social service systems, and healthcare financing.

Excellent interpersonal, communication, and crisis intervention skills.

Ability to work independently and thrive in a fast-paced environment.

Compassion, empathy, and resilience when supporting patients and families in crisis.

Commitment to confidentiality, documentation integrity, and compliance with HIPAA and professional standards.
