Position: Experienced Licensed Master Social Worker – Washington, DC

Responsibilities:

• Conduct comprehensive psychosocial assessments of patients and families to identify strengths, challenges, and support systems affecting health and well-being.

• Provide crisis intervention, counseling, and emotional support to patients and families facing medical diagnoses, end-of-life issues, and life transitions.

• Collaborate with the interdisciplinary healthcare team to develop and implement individualized care plans.

• Advocate for patients’ rights and connect them with appropriate resources such as financial assistance, housing, and community support services.

• Guide patients and families in navigating the healthcare system, medical procedures, and treatment options.

• Facilitate effective communication between patients, families, providers, and community agencies to ensure continuity of care.

• Accurately document assessments, interventions, and case management activities in compliance with regulations and policies.

• Participate in case conferences, discharge planning, and multidisciplinary team meetings.

• Remain current on social, economic, and legislative issues affecting patient care and apply knowledge in practice.

• Engage in professional development and maintain ethical, trauma-informed, and culturally competent practice.

Qualifications:

• Master’s degree in Social Work (MSW) from a CSWE-accredited program.

• Current state licensure or eligibility as an LMSW or LCSW.

• 2–3 years of clinical experience in medical social work, healthcare, or related fields.

• Strong knowledge of psychosocial assessment, counseling, and case management practices.

• Familiarity with community resources, social service systems, and healthcare financing.

• Excellent interpersonal, communication, and crisis intervention skills.

• Ability to work independently and thrive in a fast-paced environment.

• Compassion, empathy, and resilience when supporting patients and families in crisis.

• Commitment to confidentiality, documentation integrity, and compliance with HIPAA and professional standards.
